NEET Counselling Document Checklist: What You Actually Need

Every year, candidates lose medical seats at the document verification stage. They had the rank. They had the category. They had the choice. What they didn't have was the right paperwork ready at the right time.

This guide walks through every document you'll need for NEET UG counselling — both AIQ and state-specific — and explains how to prepare them in advance so document issues don't cost you a seat.

The Universal Document List

These documents apply to virtually all candidates regardless of state or quota:

NEET-Related Documents

  • NEET UG Admit Card (printed, with photograph)
  • NEET UG Score Card / Result (printed)
  • NEET Application Form Print (the original application you submitted)

Educational Documents

  • Class 10 Mark Sheet (original + photocopies)
  • Class 10 Pass Certificate
  • Class 12 Mark Sheet (original + photocopies) — most critical document
  • Class 12 Pass Certificate / Provisional Certificate
  • Migration Certificate from your Class 12 board (Indian boards) — many states require this
  • Transfer Certificate from your Class 12 school

Identity Documents

  • Aadhaar Card (original + photocopy)
  • PAN Card (original + photocopy)
  • Passport (if available — some states accept as alternative ID)
  • Voter ID (if available)
  • Birth Certificate (some states require this for age verification)

Photographs

  • Recent passport-size photographs: Multiple copies. Most counselling sessions ask for 6-10 copies. Get them taken at a professional studio with formal attire and white/light background.

Domicile / Residence Documents

For state quota counselling, domicile proof is critical:

Domicile Certificate (Most Common)

  • Issued by Tehsildar/SDM/Revenue Officer
  • Should be in the candidate's name
  • Should be valid (issued within last 1-3 years typically; check current state requirement)
  • Verify the spelling of name, father's name, address — must match other documents

State-Specific Variations

Maharashtra: Requires "Type-A/B/C/D/E" classification:

  • Type A: 15+ years residence
  • Type B: Father in state government service
  • Type C: Defense personnel
  • Type D: Other documented categories
  • Type E: Central Govt employees stationed in state

Tamil Nadu: "Tamil Nadu Nativity Certificate" specifically — different from regular domicile.

Rajasthan: "Bonafide Resident Certificate"

Uttar Pradesh: Standard domicile, may also require "Permanent Resident Certificate" depending on year

Karnataka: "Cauvery" or other regional certificates depending on local requirements

What If You Don't Have State Domicile?

If you can't establish domicile in any state:

  • AIQ counselling doesn't require domicile (apply only here)
  • Some states have alternative paths (e.g., UP allows candidates who studied 10th and 12th in UP regardless of formal domicile)

Category-Specific Documents

If you're claiming any reservation, category certificates are mandatory:

OBC Candidates

  • OBC Certificate: From Tehsildar/SDM, must specify "Non-Creamy Layer"
  • NCL Certificate: Sometimes a separate certificate required showing income < ₹8 lakhs
  • State-specific OBC (if state quota): Some states require state-specific OBC list inclusion verified
  • Validity: Most states require certificates issued within last 6-12 months. Re-issue if old.

EWS Candidates

  • EWS Certificate (Income & Asset): Specifically for EWS reservation, issued by competent authority
  • Income proof: Family income < ₹8 lakhs/year
  • Asset declarations: Per EWS rules
  • Validity: Usually 1 year

SC Candidates

  • SC Caste Certificate: From Tehsildar/SDM
  • State-specific verification: Maharashtra requires "Caste Validity Certificate" (additional certificate validating the caste certificate)
  • Permanent typically: SC certificates don't expire usually, but old ones may be questioned

ST Candidates

  • ST Caste Certificate / Tribe Certificate: From Tehsildar/SDM
  • State-specific verification: Similar to SC, some states require validity
  • Permanent typically: Don't expire

State-Specific Categories

  • Tamil Nadu: BC, BCM, MBC&DNC, SCA certificates - issued specifically by TN authorities
  • Maharashtra: SEBC, NTB/C/D, VJA - state-specific certificates
  • Rajasthan: MBC, SA - state-specific
  • UP: Minority certificates (MM/JM/BM) for minority colleges

Caste Validity Certificate (Maharashtra-specific)

If you're claiming SC, ST, OBC, or NTB/C/D in Maharashtra, you need this in addition to the basic caste certificate. Issued by state caste verification committee. Process can take months. Apply early — this is the most common reason for last-minute counselling failures in Maharashtra.

PWD (Persons with Disability) Candidates

If you're claiming PWD reservation:

  • Disability Certificate: From competent medical authority
  • UDID Card (Unique Disability ID Card): Most preferred form
  • Medical Board Certificate: If UDID not available, original medical board certificate
  • Disability percentage: Must specify the percentage
  • Type of disability: Must specify type (locomotor, visual, hearing, etc.)
  • Latest issue: Some states require recent re-certification

NEET allows specific disability types. Confirm your disability category falls within NEET's accepted list before counselling.

State-Specific Additional Documents

Tamil Nadu Specific

  • Government School Certificate (for 7.5% GS quota): From school authorities for candidates who studied Class 8 to 12 in TN government schools
  • Income Certificate: For income-based concessions
  • Tamil Nadu Community Certificate: Different from caste certificate, required for community-based seats

Maharashtra Specific

  • HSC Certificate: If Class 12 was from Maharashtra State Board (different from regular Class 12 mark sheet)
  • Caste Validity Certificate: As mentioned, separate from caste certificate
  • Type Certificate: For Type A/B/C/D/E classification

Rajasthan Specific

  • Sahariya Tribe Certificate: For SA category candidates
  • State-specific community certificates for MBC

UP Specific

  • Minority Status Certificate: For minority college applications (MM/JM/BM)
  • Resident Certificate: Sometimes required separately

Karnataka Specific

  • Cauvery Certificate or alternative state-specific certificates
  • PUC Certificate: If from Karnataka PUC board

Sports Quota Documents (if applicable)

If applying through sports quota:

  • Sports Achievement Certificates: State-level or national-level competition certificates
  • National-level participation records: Issued by recognized sports federations
  • Coaching certificates / Training records

Sports quota seats are limited and verification is rigorous. Documentation must be impeccable.

NRI Quota Documents (if applicable)

For NRI quota seats at private/deemed universities:

  • NRI Status Certificate: Proof of NRI status of family member
  • Sponsorship documents: Confirming financial support
  • NRI affidavits: Family relationship and willingness to fund education
  • Foreign address proofs: Of the NRI relative
  • Foreign income proofs: As required by institution

Extra Documents That Help

Some candidates miss these but they're useful:

Affidavits

  • Anti-ragging affidavit: Standard, signed by candidate and parent
  • Anti-drug affidavit: Some states require
  • Income affidavit: Sometimes needed for fee concessions

Medical Documents

  • Medical fitness certificate: Typically needed at college reporting
  • Vaccination certificates: COVID-19 and routine vaccinations

Bank Documents

  • Bank account details: For fee payment and any scholarship transfers
  • Loan sanction letters: If using education loan

Document Preparation Timeline

Plan backwards from counselling start dates:

6 Months Before NEET

  • Get caste certificates re-validated if old
  • Apply for caste validity certificate (Maharashtra) — slow process
  • Confirm domicile certificate validity
  • Plan PWD certification renewal if relevant

3 Months Before NEET

  • Complete domicile certificate application if not done
  • Begin EWS income/asset documentation
  • Get NCL certificate for OBC candidates

After NEET Result, Before Counselling

  • Photocopy all documents in multiple sets (5-10 copies of each)
  • Self-attest documents as required
  • Notarize documents if needed
  • Prepare digital scans for online registration
  • Verify spellings and names match across all documents

During Counselling

  • Carry originals to verification
  • Carry multiple photocopy sets
  • Have parents/guardians on standby for any signing
  • Keep digital backups in cloud storage

Common Mistakes That Cost Seats

These errors cost candidates real seats every year:

1. Spelling Discrepancies

Your name in NEET application: "Rahul Kumar Sharma" Your domicile certificate: "Rahul K. Sharma" Result: Document verification flags inconsistency. May reject or require affidavit.

Fix: Verify all documents have IDENTICAL name spelling. If not, get an affidavit with name correction.

2. Missing Caste Validity (Maharashtra)

Have caste certificate but not caste validity → Reservation benefits invalidated for state quota.

Fix: Apply for caste validity months in advance.

3. Expired Certificates

Caste certificate from 5 years ago → Some states require recent issue.

Fix: Get fresh certificate issued before counselling.

4. Wrong Type of Certificate

Maharashtra Type-A claim with general domicile (no Type classification) → Insufficient documentation.

Fix: Get the specific certificate format your state requires.

5. Forgetting Migration Certificate

Class 12 from CBSE/ICSE board, no migration certificate → Some state counselling rejects without it.

Fix: Apply with your previous board for migration certificate.

6. Photocopies Not Self-Attested

Submitted photocopies but not signed by candidate → Some states require self-attestation.

Fix: Sign each photocopy with "Self-attested" annotation and your signature.

7. Missing Financial Proof

For EWS or fee concessions, missing income proof → Reservation/concession denied.

Fix: Maintain income certificate, ITR, and bank statements ready.

8. NEET Application Photo Doesn't Match Recent Photo

Substantial appearance change between NEET application and counselling photo → Some states question identity.

Fix: Carry both old and recent passport photos, plus original ID with photo.

9. Online vs Offline Verification Issues

State requires physical verification but candidate uploads only → Lost time scheduling re-verification.

Fix: Read state notification carefully for online vs offline document submission.

10. Last-Minute Discovery

"I need a caste validity certificate?" — discovered 3 days before counselling → Impossible to get in time.

Fix: Read counselling notification thoroughly. Make complete checklist. Verify everything weeks before deadlines.
