Bihar runs 13 government medical colleges with a combined annual intake of approximately 1,645 MBBS seats, including 1 AIIMS Patna (Central, AIQ-only) and 1 ESIC Bihta (Central Govt). The 11 state government colleges span heritage institutions (Patna Medical College established 1925, Darbhanga Medical College 1946), the 1970s expansion (ANMMC Gaya, Nalanda MC Patna, SKMC Muzaffarpur, JLNMC Bhagalpur), and a 2011 to 2023 wave (IGIMS Patna, VIMS Pawapuri, GMC Bettiah, JKTMCH Madhepura, GMC Purnea). State govt seats are filled by BCECEB through the 85% state quota; 15% AIQ seats are filled by MCC.
The flagship Bihar government medical colleges are AIIMS Patna (2012, AIQ-only via MCC), Patna Medical College & Hospital (PMCH, 1925, 200 seats, oldest), Darbhanga Medical College (DMC, 1946, 120 seats), Indira Gandhi Institute of Medical Sciences (IGIMS, 2011, 150 seats, Deemed Univ status), and Nalanda Medical College Patna (NMCH, 1970, 150 seats).
